     Integrated Health Care

    • The care a patient experiences as a result of a team of primary care and behavioral health clinicians, working together with patients and families, using a systematic and cost-effective approach to provide patient-centered care for a defined population.
    • This care may address mental health and substance abuse conditions, physical health behaviors (including their contribution to chronic medical illnesses), life stressors and crises, stress-related physical symptoms, and ineffective patterns of health care utilization.
    • Whole person, patient centered care. It looks at a more holistic way of treating physical and mental health needs in a PCP setting. Each patient will have a care team assigned to ensure continuity of care.
    • Care Team: A care team can consist of at a Medical Practitioner, Behavioral Health Professional, Integrated Case Manager, Integrated Care RN, Medical Assistants and those providing ancillary and diagnostic services. Care teams are site specific. They manage individual and population patient care as a team effort, working together to enhance the care provided to patients throughout their lifetime to maximize health outcomes.

    CBI’s services may include:

    1. Regular Check-Ups and Well Person Visits
    2. Well Women Exams (Site Specific)
    3. Same day appointments (for emergent and urgent care)
    4. Age appropriate screenings
    5. Developmental screenings (site specific)
    6. Well Child (site specific)
    7. Sports Physicals
    8. Minor Surgical Procedure
    9. Diabetes Management: Medication Management and/or holistic approach
    10. Vaccinations
    11. Counseling/Wellness Groups
    12. Blood Pressure Management Medication Management and/or holistic approach
    13. Cholesterol Management
    14. Pregnancy Testing
    15. Treatment of Minor emergencies including laceration repair and skin wound care
    16. Dermatology Services: Basic Skin Checks
    17. Mental Health Screening and Treatment

    21. Chronic Care Management (including self-care)
