Assertive Community Treatment

Assertive Community Treatment, or ACT, is an evidence-based practice of delivering a full range of services to people who have been diagnosed with a serious mental illness. ACT’s goal is to give patients adequate community care and to empower patients to live independently in the community.

CBI facilitates 3 Forensic Assertive Community Treatment teams (F-ACT) as well as one Assertive Community Treatment Team (Act). CBI ACT team has mutual expectations between the team and its patients that are met collaboratively. These expectations include face to face engagements at least 4 times per week, creating and developing support systems, maintaining home visits, all in an effort to help identify and work towards patient goals.


Honoring our recovery-based origins, Community Bridge, Inc. Assertive Community Treatment (ACT) teams will be made of up individuals who have backgrounds as Peer Support Specialists each possessing unique experiences with the homeless and forensic population.
